Home Modifications Assessment

Home modifications can transform your living space — making it safer, more accessible and enabling you to continue living independently at home. Carmen conducts detailed assessments of your home environment, creates a scope of works with clear recommendations, and prepares all the NDIS documentation needed to get your modifications approved and funded.

Types of home modifications

Modifications range from simple, low-cost changes that make a big difference, to complex structural renovations. Carmen assesses what's right for your specific needs:

Simple Modifications (low cost)

  • • Grab rails and handrails in bathrooms and hallways
  • • Hand-held shower hoses and slide bars
  • • Non-slip flooring and bath mats
  • • Lever-style door handles and tap extensions
  • • Raised toilet seats and toilet frames
  • • Kitchen bench height adjustments

Complex Modifications (higher cost)

  • • Accessible bathroom renovations
  • • Shower and toilet level floor ramps
  • • Widening doorways for wheelchair access
  • • Ramp installation at entry points
  • • Lowering kitchen benchtops and cabinets
  • • Ceiling hoist installations

How does the assessment process work?

Carmen takes a methodical approach so that every modification recommendation is practical, well-documented, and NDIS-ready:

  1. Home visit: Carmen visits your home to walk through each room and identify barriers to safety and independence. She'll note measurement details, observe how you move through the space, and understand your daily routines.
  2. Needs analysis: Based on the visit, Carmen maps out which modifications are most important — prioritising safety risks first (like fall hazards in bathrooms) then addressing longer-term accessibility goals.
  3. Scope of works: You receive a detailed report listing each recommended modification with clear specifications so builders and tradespeople know exactly what to do. The report includes photos, measurements and reference standards.
  4. Funding application support: Carmen prepares the NDIS justification documentation for each modification — including clinical rationale, cost estimates (where available), and evidence of why each change is reasonable and necessary.
  5. Site visits and quality assurance: For larger projects, Carmen can attend site during works to ensure the modifications are being installed correctly and meet your needs. This is especially valuable for bathroom renovations and structural changes.

Why an occupational therapist for home modifications?

While builders and tradespeople are excellent at executing works, it's the occupational therapist who understands how your disability affects daily living. Carmen combines clinical knowledge with practical awareness of Australian building standards to recommend modifications that aren't just compliant — they actually work for you.

Many people assume "a grab rail is just a grab rail," but the height, placement and type of grab rail can mean the difference between feeling secure in the shower or needing help for every wash. Carmen's experience across dozens of homes means she knows what to specify.

How NDIS funding works for home modifications

The NDIS funds home modifications under the "Improved Living" capacity building budget and the "Capital Supports" budget, depending on cost:

Minor Modifications (up to $1,000 per request, $2,000 total per plan year): These can be approved directly by the NDIA without a full assessment for simple items like grab rails and shower chairs. Carmen's reports make the process fast.
Living Layout modifications (up to $15,000): Moderate works like bathroom or kitchen alterations that don't change the building's structure. Requires a professional assessment — this is where Carmen's report is essential.
Home modifications (over $15,000): Major structural changes like widening doorways for wheelchair access or building ramps. Requires a detailed OT assessment and scope of works from Carmen, plus approval through the NDIA's Capital Supports pathway.
