Highly experienced occupational therapist for Functional Capacity Assessments, Assistive Technology and Home Modifications. Helping adults live independently.
This is a comprehensive assessment of your ability to perform the activities you need and would like to do in your day to day life. The report provided will identify strengths and limitations, and will include recommendations for the supports needed to enhance your safety and independence. This may include assessment for Supported Independent Living (SIL), Individualised Living Options (ILO) or Specialist Disability Accommodation (SDA) supports if applicable.
Assistive Technology refers to devices or equipment that can help a person with a disability perform a task that they might otherwise be unable to do. This can range from simple items such as a basic shower chair to more complex items such as wheelchairs or scooters. An AT assessment will include a review of your needs, the environment where the AT will be used, trial of equipment and the completion of any necessary documentation required to support NDIS funding of the item.
Home modifications refer to changes to your home to improve accessibility, independence and safety. Home modifications can range from simple changes such as a grab rail or hand held shower hose, to more complex works like creating an accessible shower. This service will include an assessment of your needs and your home environment, creating a scope of works, arranging site visits and quotes for the works, and completing the relevant NDIS required documentation.

Common questions about occupational therapy and NDIS services. If you don't see your question here, get in touch and I'll be happy to help.
A Functional Capacity Assessment (FCA) evaluates your ability to complete everyday activities — from self-care tasks like bathing and cooking, to communication, mobility, and social participation. FCAs are commonly needed for NDIS plan reviews, Supported Independent Living (SIL) applications, or when changes to your disability affect your daily functioning. Carmen's FCA reports are widely accepted by NDIA planners and are designed to clearly present your strengths and support needs.
Yes, if the service is considered reasonable and necessary under your NDIS plan. Most NDIS plans include funding for allied health services including occupational therapy. Carmen works with self-managed and plan-managed participants directly — no extra paperwork needed for plan managers. You'll need to check that 'Improved Daily Living' or 'Core Supports' budget items in your plan cover OT assessments.
Assistive Technology (AT) covers equipment and devices that help with daily tasks — from shower chairs and grab rails to wheelchairs, scooters, and computer access software. If a device would help you perform tasks more safely and independently, it's likely to be NDIS-funded. Carmen assesses your needs, reviews your living environment, trialsequipment, and prepares the documentation required for NDIS approval.
Carmen visits your home to identify barriers to safety and independence — then creates a detailed scope of works with recommended modifications. These range from simple changes like grab rails, hand-held shower hoses and ramp installation, to more complex works like creating an accessible bathroom or widening doorways. The report includes cost estimates, helps you apply for NDIS funding, and can guide builders during the modification process.
Absolutely. Carmen primarily works with self-managed and plan-managed NDIS participants. For self-managed participants, you pay the invoice directly and claim it through your NDIS portal. Plan-managed participants can use their support coordinator or plan manager to engage Carmen's services — there are no extra fees or administrative hurdles.
